MOSNET REMOTE MEMBER SERVICE AGREEMENT 1. MOSNET SERVICES ARE PROVIDED BY: MOSAIC COMMUNICATIONS, INC., (MOSCOM) 401 CENTERPOINT BLDG., ORTIGAS COMPLES PASIG, METRO MANILA. 2. Internet protocols provide end-to-end verification of the accurate receipt of messages, but MOSCOM makes no attempt to verify accurate receipt and is not responsible for any loss of data resulting from delays, non-delivery, incorrect delivery, or service interruptions, including those caused by the negligence, errors or omissions of MOSCOM, nor for consequential damages regardless of their cause. 3. RATE SCHEDULE: The following monthly service fee is for the first 20 hours of connect time. MOSNET REMOTE : p1,000 MOSNET REMOTE/IP : p2,500 additional hours are billed in one hour increments at the rate of : p75 per hour. 4. SET-UP CHARGES: The following one-time set-up charges apply: MOSNET REMOTE : p3,000 MOSNET REMOTE/IP : p5,000 5. Payment is due upon receipt of the monthly invoice. Failure to pay when due may result in suspension or discontinuance of service. A service charge may be imposed for reactivating discontinued service. 6. This agreement shall be governed by, and construed in accordance with the laws of the Philippines and will remain in force until terminated by writing from either party. The termination date will be the end of the month the termination notice is received. Customer is obligated to pay through the termination date, as specified on the invoice. 7. Applicant's signature constitutes agreement to comply with the conditions of this agreement and the guidelines on the opposite page governing the use of the network connection. Failure to follow these guidelines will result in discontinuance of service. Guidelines subject to change.
